Event Form

If you would like to have an event at Christian Life Church, we ask that fill out this event form below and bring the appropriate deposit necessary to the church office to reserve the date for your event.

*All events have to be submitted 3 weeks prior to the day of your event.
Guidelines
1. You will be responsible for any dirty dishes, emptying trash into the dumpster located outside of the building, making sure all personal belongings are removed from the building immediately following your event, and being certain the facilities are left orderly (excluding janitorial duties). We are not responsible for any personally belongings.

2. No receptions are to be held on Sundays.

3. Nothing is to be hung on the walls for any reason. No tape, tacks, nails, etc.

4. Church tablecloths are not available for use.

5. Smoking is forbidden - strictly prohibited.

6. No intoxicants of any kind are to be served in connection with the event or used on the church campus at any time.

7. You are responsible for your own supplies.
